Thebehavioursunderquestionaremultifactorialinorigin.Therearefamilial,religious,andculturalvaluesthatareacquiredlongbeforemedicalschool.Forexample,countries,cultures,andsubculturesexistwherebribesanddishonestbehaviourarealmostanorm.Therearesecondaryschoolsinwhichneitherstaffnorstudentstoleratecheatingandotherswherecheatingisrampant;therearehomeswhichimbueyoungpeoplewithhighstandardsofethicalbehaviourandotherswhichleaveethicaltrainingtotheharmfulinfluenceoftelevisionandthemarketplace. Medicalschoolsreflectsocietyandcannotbeexpectedtoremedyalltheillsofasociety.Theselectionprocessofmedicalstudentsmightbeexpectedtofavourcandidateswithintegrityandpositiveethicalbehaviour--ifonehadareliablemethodfordetectingsuchcharacteristicsinadvance.Medicalschoolsshouldbethemajorfocusofattentionforimbuingfuturedoctorswithintegrityandethicalsensitivity.Unfortunatelytherearetroubling,ifinconclusive,datathatsuggestthatduringmedicalschooltheethicalbehaviourofmedicalstudentsdoesnotnecessarilyimprove;indeed,moraldevelopmentmayactuallystoporevenregress. Thecreationofapervasiveinstitutionalcultureofintegrityisessential.Itiscriticalthattheacademicandclinicalleadersoftheinstitutionsetapersonalexampleofintegrity.Medicalschoolsmustmaketheirinstitutionalpositionandtheirexpectationsofstudentsabsolutelyclearfromdayone.Thedevelopmentofaschool'scultureofintegrityrequiresapartnershipwiththestudentsinwhichtheyplayanactiveroleinitscreationandnurturing.Moreover,theschool'sexaminationsystemandgeneraltreatmentofstudentsmustbeperceivedasfair.Finally,thetreatmentofinfractionsmustbefirm,fair,transparent,andconsistent.
